To provide you the best health care possible and to make the most of our time together, it is important for you to prepare in advance for your visit. We want this to be a positive experience for you, therefore, follow the steps below to ensure your visit goes as planned.

INSURANCE & ID CARD

Please bring your health insurance and ID card.

PAYMENT

You are responsible for the co-payment, co-insurance and/or deductible at the time services are rendered. Visa, Master Card, checks or cash are accepted. We welcome all questions you may have regarding your fees, account balance, or insurance payments.

MEDICATIONS

Please bring all medications that you are currently taking including spacers and over the counter medications.
