What to Bring

To provide you the best health care possible and to make the most of our time together, it is important for you to prepare in advance for your visit. We want this to be a positive experience for you, therefore, follow the steps below to ensure your visit goes as planned.

Patient Forms

New Patients: Please print and complete the forms located below. Submit the completed documents when you check-in at the receptionist’s desk.

Insurance & ID Card

Please bring your health insurance and ID card.

Payment

You are responsible for the co-payment, co-insurance, and/or deductible at the time services are rendered. Please note that as of April 2nd, 2026, AllergySA imposes a surcharge of 3% on the transaction amount on Visa, Mastercard, and Discover credit card products, which is not greater than our cost of acceptance. We do not surcharge debit, HSA, FSA, or prepaid cards.

We welcome all questions you may have regarding your fees, account balance, or insurance payments.

Medications

Please bring all medications that you are currently taking including spacers and over the counter medications.

Medical Records

If you have them, great bring them. If not, we’ll request them.
