Referral Requirements:

• If your insurance plan requires a referral from your Primary Care Provider (PCP) before you can see a specialist, please contact your PCP to request a referral before scheduling your appointment.
• Please note that without a referral on file, your insurance may not cover the visit, and you may be responsible for the full cost of the visit.  To avoid delays or rescheduling, please confirm your referral has been processed prior to your appointment date.
• If you are unsure whether your plan requires a referral, please contact your insurance company directly. If you have additional questions, give us a call at (210) 616-0882.

If you have not been seen in one of our offices within the past three (3) years, please schedule as a New Patient.
